Neck Pain and Shoulder Pain: Why It Happens and How Physical Therapy Fixes It

Neck pain and shoulder pain are two of the most common reasons people walk into our clinic. Sometimes the pain shows up suddenly after sleeping in a bad position. Other times it builds slowly over weeks or months until turning your head or reaching overhead becomes a real problem. Either way, neck pain treatment through physical therapy is one of the most effective ways to get lasting relief and prevent the problem from coming back.

What Causes Neck Pain?

The neck is one of the most mobile parts of your spine, which also makes it one of the most vulnerable. It supports the weight of your head (roughly 10 to 12 pounds) and allows you to look up, down, and side to side throughout the day. When something goes wrong in this area, you feel it quickly.

Here are the most common causes of neck pain we see in our clinics:

  • Muscle strain and tension. This is the most frequent cause, often triggered by poor posture, prolonged screen time, or sleeping in an awkward position. The muscles on the sides and back of the neck become tight, sore, and sometimes go into spasm.

  • Cervical disc herniation. When a disc in your neck pushes out of alignment, it can press on a nerve root and cause pain that radiates into the shoulder, arm, or hand. This is often accompanied by numbness or tingling.

  • Cervical stenosis. Narrowing of the spinal canal in the neck that puts pressure on the spinal cord or nerve roots. This is more common in adults over 50.

  • Whiplash. A sudden back-and-forth motion of the head, most commonly from car accidents, that damages the soft tissues of the neck.

  • Degenerative changes. Arthritis and normal wear on the cervical spine joints and discs can cause chronic stiffness and pain over time.

Many patients come in thinking they just have a "stiff neck" when the underlying cause is actually a disc issue, a nerve compression, or a postural problem that has been developing for years. That is why a proper evaluation matters.

What Causes Shoulder Pain?

The shoulder is the most mobile joint in your body, and that freedom of movement comes at a cost. The structures that hold your shoulder together, the rotator cuff tendons, labrum, bursa, and ligaments, are under constant stress, especially if you are active or do repetitive overhead work.

Common causes of shoulder pain include:

  • Rotator cuff injuries. Tears, strains, and tendinitis of the rotator cuff muscles are extremely common, particularly in people over 40 and athletes who throw or swim.

  • Frozen shoulder (adhesive capsulitis). A condition where the shoulder capsule becomes thick and tight, severely limiting your range of motion. It often develops gradually and can take months to resolve without treatment.

  • Shoulder impingement. When the rotator cuff tendons get pinched between the bones of the shoulder during overhead movements, causing pain and inflammation.

  • Labral tears. Damage to the ring of cartilage that surrounds the shoulder socket, often from sports injuries or repetitive motions.

  • Referred pain from the neck. This is something many patients do not realize. A problem in the cervical spine can send pain directly into the shoulder, mimicking a shoulder injury when the real issue is in the neck.

That last point is important. Neck and shoulder pain frequently overlap, and treating the wrong area will not give you results. Our therapists are trained to identify whether your shoulder pain is coming from the shoulder itself, from the neck, or from both.

How Physical Therapy Treats Neck and Shoulder Pain

Physical therapy is the recommended first-line treatment for most neck and shoulder conditions. It works because it does more than just reduce pain. It corrects the underlying problem so the pain does not keep returning.

Manual Therapy and Joint Mobilization

Your therapist uses hands-on techniques to restore normal mobility in the joints of your cervical spine and shoulder. This includes gentle joint mobilizations, soft tissue release, and myofascial techniques that reduce muscle guarding and improve range of motion. Targeted Strengthening

Once your pain is under control, we progressively strengthen the muscles that support your neck and shoulder. For neck pain, this means building up the deep cervical flexors and upper back stabilizers that keep your head in proper alignment. For shoulder pain, we focus on rotator cuff strengthening, scapular stabilization, and restoring normal shoulder blade mechanics.

Posture Correction

Poor posture is one of the biggest drivers of neck and shoulder pain, especially for people who spend long hours at a desk, on a computer, or looking down at a phone. For chronic shoulder conditions like calcific tendinitis, persistent rotator cuff tendinopathy, and frozen shoulder, shockwave therapy uses acoustic waves to stimulate healing and break down scar tissue. It is performed in-office and typically requires 3 to 6 sessions for optimal results.
